FL Continuing Education Self-Submission Information

If you are a FLORIDA TECHNOLOGIST and must self-submit, please see the information below.

The Florida Bureau of Radiation Control will receive a monthly report from us if the course has an assigned FLDOH-BRC reference number. If there is no FLDOH-BRC reference number and “self-submit” is on the certificate of completion, please follow the self-submit instructions below.

To Self-Submit the technologist can:

UPLOAD the CE certificate with their renewal application at www.flhealthsource.gov
EMAIL to MQAonlineservice@flhealth.gov
FAX to 850-245-4791 or
MAIL to MQA Licensure Support Services (address below).
